All student athletes in the Moore district will need the following:
  1. Completed yearly Physical Evaluation by a physician.
  2. Completed 'OSSAA Eligibility Record Form.'
  3. Medical insurance or waiver on file.
  4. Completed 'Emergency Contact Card.'
  5. Concussion Awareness Sheet  Parental Consent to Participate
