• ENROLLMENT INFORMATION/REQUIREMENTS
     
     
    KINDERGARTEN
    • All children who will be 5 on or before September 1st of the current year, should enroll.
    • A state certified birth certificate and two "proofs of residence" are necessary for enrollment.
    • Oklahoma State Law requires that all 5-year old children entering school for the first time must show proof of the following vaccines:
                5 DTP,   4 Polio,  2 MMR, 3 Hepatitis B*,  2 Hepatitis A*,   1 Varicella (chicken pox)
     
    *Hepatitis A is a 2 dose series and Hepatitis B is a 3 dose series.  They require 4-6 months to complete.
     
    GRADES 1-6
    • All children in grades 1-6 should provide a state certified birth certificate and two *proofs of residence for enrollment.  
    •  Oklahoma State Law requires that all children should show proof of the following vaccines:

    ·          IMMUNIZATIONS

     
    State Law (effective 6-18-98) requires that:   By law there is no longer a grace period for transfer, lost and/or unavailable records.  In these incidents the parents, by law, will be referred to their personal doctor or county health department for reconstruction of the immunization records.

              *PROOF OF RESIDENCE (2 REQUIRED)

     
            Information or documentation to prove student residency in the School District shall include proof of provisions of utilities (gas, water or electric only), payment of ad valorem taxes, local agreements or contracts for purchasing/leasing housing, or mortgages.

     

    We can accept water, gas or electric bills issued within the last two months before a student’s enrollment    WE CANNOT ACCEPT CABLE OR PHONE BILLS, ETC.  We can accept a lease agreement on a residence if it has been signed within 30 days of enrollment for one of the proofs of residence.  and signed by all parties may also be accepted.A purchase contract on a home showing the date signed, address, completion date
