  • Employees cannot elect a dental plan without first having proof of some other
    GROUP medical plan. 
  • Dependents cannot be on a plan that is different from the employee's plan.
  • Employees may add dependents; cover one cover all rule; all children must be
    covered uless they can prove other GROUP dental coverage.  A spouse may
    be excluded from dental when children are being covered by completing a
    Spousal Exclusion Form.
