**Cost for current students only: .25 cents per transcript

TRANSCRIPTS REQUEST INFORMATION
**Under State and Federal laws, no other party may pick up someone else’s transcript including parents, siblings, wives/husbands, etc if the person is over 18 and not currently enrolled at MHS. A photo ID must be presented when transcript is picked up.

**NO GRADUATION VERIFICATIONS WILL BE GIVEN OVER THE PHONE PER THE FERPA LAW.

FOR FORMER STUDENTS TO ORDER A TRANSCRIPT:

We do NOT have copies of diplomas. The legal document for proof of attendance/graduation is the official transcript.

*All requests must be made in writing, either in person in the Main Office, or by mail.

Please include the following:

*Full name that you were enrolled under
*Date of birth

*Year graduated or year dropped

*Your Signature

*$1.00 Cash, check or money order made out to "Moore High School"

*Self-addressed, stamped envelope to where transcript is to be mailed.

*Mail to: Registrar, Moore High School

**Money must be received before order will be processed

FOR VERIFICATION COMPANIES:
All requests must be made through the mail with a $5.00 fee and the following info:

*client's signed release

*name enrolled under

*date of birth

*year of graduation

Your transcript will be ready for pick up in the Main Office, 24 hours after request is received, Monday through Friday, from 1:00 pm to 3:00 pm.
