Pre-enrollment Information for Kindergarten and Grades 1 thru 6th:
Pre-enrollment for grades Kdg thru 6 will be April 23rd thru 27th
If your student is new to Moore Public Schools you will need the following information:
The following four items must be presented at enrollment:
1. Under Oklahoma State Law, a document for verification of birth is needed for kindergarten and first grade. Birth certificates can be obtained at the Division of Vital Records, Oklahoma State Department of Health, 1000 N.E. 10th, PO Box 53551, OKC, OK 73152. (short form birth certificate is unacceptable)
2. Oklahoma State Law requires immunizations as stated below in Board Policy # 7010. State Law (effective 6-18-98) requires that: By law there is no longer a grace period for transfer, lost and/or unavailable records. In these incidents the parents, by law, will be referred to their personal doctor or county health department for reconstruction of the immunization records.
3. Name and address of previous school for record release form.
4. Information or documentation to prove student residency in the School District shall include but not be limited to proof of provisions of utilities (gas, water or electric only), payment of ad valorem taxes, local agreements or contracts for purchasing/leasing housing, or mortgages.
5. Students enrolling on an affidavit will need to enroll beginning on the first day of fall enrollment in August. Immunization records, birth certificate and the person signing the affidavit must provide two proofs of residency. Proofs of residency shall include but not be limited to proof of provisions of utilities (gas, water or electric only), payment of ad valorem taxes, local agreements or contracts for purchasing/leasing housing, or mortgages.
The affidavit will become null and void if the student moves from the affidavit address. A new affidavit must be completed each year.
Immunization Information: According to Oklahoma School Immunization law all children must present upon school entry a certified immunization record indicating the date and type of immunization received or must present proof that such child is in the process of receiving required immunization(s).
The immunization requirement and the frequency of their administration is prescribed by the State Board of Health.
Exemptions to the immunization law may be granted in accordance with the state law and State Health Department requirements.
