Once you have contacted me (in person if possible!), please fill out the following form Letter of Rec Info Form. You must log in using your district email and password.
I also need a copy of your transcript. You may email it to firstname.lastname@example.org or bring me a hard copy. It does not need to be the official version.
Once I have written a letter of recommendation, i will keep it on file for two years. If you need me to adapt it for another application, scholarship, etc., please email me. You do not need to re-submit the Letter of Rec Info Form.
