Moore Public Schools requires a medication consent form for medication to be given at school. This includes both prescription and non-prescription medications. If your child has an asthma, anaphylactic or diabetic medication that he or she may carry and self administer, please fill out the second page for permission to do so. Per Moore Public School Policy, we cannot administer herbal or homeopathic medication at school.
Use this plan if your child takes ADD or ADHD medication at school.
