Residency Affidavits

Beginning with the 2026–2027 school year, Moore Public Schools will replace the Residency Affidavit process with a new Co-Residency Verification process.

Co-Residency Verification is required when a student and their parent/legal guardian reside with another individual and do not have an approved proof of residency in the parent/legal guardian's name.

Who Must Complete Co-Residency Verification?

Co-Residency Verification is required when:

  • The student and parent/legal guardian live together in the residence of a Moore Public Schools district resident.
  • The parent/legal guardian does not have a qualifying utility bill, lease agreement, or other approved residency document in their own name.

Where Can Verification Be Completed?

  • New students must complete Co-Residency Verification at the Enrollment Center.

Returning students renewing their verification for the upcoming school year may complete the process at either the Enrollment Center, beginning June 1 each year, or at their home school once school staff has returned before the start of the new school year.

Required Attendance

Both of the following individuals must appear in person together:

  • The student's parent/legal guardian
  • The resident with whom the family is living

Both parties must present a valid Oklahoma driver's license or Oklahoma state-issued ID.

Required Proof of Residency

The district resident must provide one of the following documents:

Utility Bill

  • Gas, water, or electric bill
  • Must be dated within the last 30 days
  • Must include the customer name and service address
  • The payment/remittance portion of the bill will not be accepted

Lease or Rental Agreement

  • First page showing the resident's name, address, and lease terms
  • Signature page signed by both the tenant and landlord

Annual Requirement

Co-Residency Verification must be completed each year before school starts.

Important Information

  • The student and parent/legal guardian must reside together with a Moore Public Schools district resident.
  • Verification becomes invalid if the family moves from the verified address.
  • A new verification must be completed any time the family's living arrangement changes.
